For the third grade, moderate to severe acne skin falls into this category when numerous papules and pustules along with occasionally inflamed nodules. If the affected area consists of the chest and back of the body, it may fall under the third category. Lastly, for the fourth tier, which is the severe acne normally consists of nodulocystic which is large, painful and inflamed pustules and nodules which form within the skin.

Acne can be unnoticeable to some, depending on the severity and location while others would consider them as skin diseases. Acne can appear on the face, back of the body, chest or basically we can say, anywhere on our skin. As scary as it sounds, acne also happens because of various factors such as hormonal changes, genetics, diets and stress. For the one who is in early adolescence, the skin tends to have acne due to having an imbalance hormone which is called "adult acne". For adults that have already passed their age of having haywire hormones, it is not well known why certain people still continue developing acne.
