For second grade which consists of moderate acne, are the multiple papules and pustules. For the third grade, moderate to severe acne skin falls into this category when numerous papules and pustules along with occasionally inflamed nodules. If the affected area consists of the chest and back of the body, it may fall under the third category. Lastly, for the fourth tier, which is the severe acne normally consists of nodulocystic which is large, painful and inflamed pustules and nodules which form within the skin.
